My Sim just got promoted to Resident (Medical 5) and when I check his Career screen he has all the weekdays as work days, and today is thursday. However, it says his next work day is in 4 days. Is this normal?

It's normal for them to not have to go to work the same day that they get a promotion. If that's what's happened, it would explain why he doesn't have to work today. But he should still have to be at work on his next scheduled work day - he shouldn't have 4 days off. Sounds like a glitch to me.

I think it is Thursday in-game, but AFTER the promotion. Which means he would have Friday off, which would result in having the next 3 days off. And that means that indeed, in 4 days your Sim will have to work again.

Well he got married the day before the promotion, and then his wife got pregnant during the weekend. Now the thing that says when your next work day is fixed but the carpool never comes, and he never has the "missing work" moodlet...
His medical beeper works though and he was called twice.
